New Patients

We are happy to register you as a patient at Chippenham Surgery if you live within the boundary of our practice area. Please contact the Surgery for details of our practice area.

You can only be a registered patient with one GP surgery, so if you register with us you will be de-registered with your current GP surgery and your previous medical notes will be sent to us.
